  • B.F Deduction Proof
  • Death Certificate issued from Union Council
  • If Govt. Employee Invalid retired then Invalidation Letter issued from D.G. Health, Punjab

  • Non Remarriage Certificate from Widow, attested by Union Council Nazim
  • Death Certificate during service issued by Head of Department
  • If Death after Retirement then Retirement Order or Pension Book Copy
  • Non Remarriage Certificate from Widower if employee is Women, attested by Gazetted Officer
  • In case of Minors then his/ her Birth Certificate from Union Council and Guardian Certificate from Guardian Court
  • Family Members' List with Name, Date of Birth, Relation (with deceased), Martial Status and Profession attested by Head of Department and attestation there is no other family member
